Common Terminology Service Version 2 (CTS2) Specification Profile


Introduction

The Common Terminology Service Version 2 Functional Model (CTS2 SFM) is a functional standard defining the capabilities, responsibilities, inputs, outputs, and expected behavior defining a set of core functionality to support the management, maintenance, and interaction with ontologies and medical vocabulary systems.

The ability to share, query and maintain value sets (or other terminology artifacts) using an accepted industry standard service interface will allow standard terminology content to be readily disseminated and validated. The need for such an interface becomes more critical as organizations begin to undertake the enhancement and maintenance of terminologies to support alternate language translations, jurisdictional extensions to standard terminologies, or maintenance and development of local terminologies. Currently, these organizations employ a variety of tools and technologies to manage terminologies and value sets.

Common Terminology Services – Release 2 (CTS 2) provides a consensus API that can be integrated over top of existing terminology systems, allowing disparate terminology systems to interoperate via CTS 2 interfaces while maintaining and fulfilling their local, often specific, terminology functional requirements.

With formal support from 14 separate organizations – including (but certainly not limited to) 3M Health Information Systems, Intermountain Healthcare, IHTSDO, Mayo Clinic, NCI Enterprise Vocabulary Services, and the University of Oxford, Department of Computer science – CTS 2 encompasses the best practices and ideas of the terminology management and user community.

CTS2 Derivative Work


The CTS2 Development Framework is a development kit for rapidly creating CTS2 compliant applications. The Development Framework allows for users to create plugins which may be loaded into the Development Framework to provide REST Web Services that use CTS2 compliant paths and model objects. Because the Development Framework is plugin based, users are required only to implement the functionality that is exclusive to their environment. For example, in any REST service, a large portion of code must be written to accept HTTP requests, handle errors, accept parameters, etc. The Development Framework provides all of this infrastructure, as well as utilities to help create plugins. The developer then is left to create the implementation plugin, which is the code specific to the individual environment.

The Standard Terminology Service (STS) was developed by Phast -- a terminology provider and distributor in hospital Pharmacy Medication Terminology (CIOsp) and Medical Devices Terminology – (CIOdm) -- using proprietary means. The natural course of Phast's parallel activities is to find a mechanism of managing these terminologies in a more efficient manner, switching to web services and providing a more robust and internationally-aligned mechanism. The Standard Terminology Service (STS) provides terminologies for the templates used in the CDA-based National Electronic Health Record (Dossier Médical Personnel) as mandated by the French government through ASIP Sante . The terminologies used are diverse, based on the national adoption of the international terminologies such as LOINC, HL7 Vocabularies, ICD-10 (French), EDQM (French), SNOMED 3.5 (French), PathLex, IHE-based vocabularies, and French national terminologies.
